A paradigm for the environmental oncology prevention and treatment base: Lanzhou University (Gansu) Model based on the systematic construction of cross-dimensional integration

Abstract
Environmental factors play a crucial role in the initiation and progression of tumors. To conduct in-depth research on the environmental factors of tumor initiation and progression, and their prevention and treatment strategies, the Environmental Oncology Committee of the China Anti-Cancer Association has established research bases for the prevention and treatment of environmental oncology across the country, aiming to enhance the level of tumor prevention and control. The construction of environmental oncology prevention and treatment bases adopts an interdisciplinary integrated approach involving epidemiology, big data, basic medicine, clinical medicine, and environmental science. It focuses on environmental factors related to tumor initiation and their mechanisms, combining macroscopic environmental factors with microscopic molecular changes to explore and develop new approaches for the prevention and treatment of environmental oncology. Taking the "Lanzhou University (Gansu) Model, " the earliest established environmental oncology prevention and treatment research base in China, as a paradigm, this paper introduces the construction goals of the environmental oncology prevention and treatment base, and elaborates in detail on four key elements: the large clinical database for environmental oncology, the early warning and monitoring system for high-risk populations, early screening, diagnosis, and treatment technologies, and the new model for prevention and control. The "Lanzhou University (Gansu) Model'' highlights interdisciplinary collaboration and the establishment of research centers, and establishes a cross-dimensional systematic framework of "data-driven-early screening and warning-precise intervention" and a five-level prevention and control system across "provincial–municipal–county–township-village". The implementation of this model has led to a 2.85% reduction in the incidence rate of digestive system tumors in high-incidence areas. The "Lanzhou University (Gansu) Model", as a precise prevention and control strategy for tumors, is being promoted nationwide in China. Meanwhile, the challenges in the prevention and treatment of environmental oncology such as environmental exposure assessment, data integration, research methods of environmental oncology, and talent development have also been identified. The prospects for the promotion and application of the "Lanzhou University Model" are also discussed.
