A study on frequency and pattern blood transfusion among sickle cell disease patients attending a tertiary care hospital of Raipur city (C.G.), India

Abstract

Background: Sickle Cell Anemia is the most prevalent inherited blood disorder worldwide. Characteristic complications of SCA are the result of the hemoglobin polymerization and sickling shape of the red blood cells. Sickled red blood cells are rigid, sticky and elongated, increasing vasoocclusion as well as a causing a hemolytic state due to the decreased lifespan of these cells. Currently there are two main therapeutic options in the management of Sickle Cell Disease i.e. hydroxy-carbamide and Blood Transfusion. Although, the clinical benefits of Blood Transfusion for patients with sickle cell disease have long been recognized, complications associated with transfusion have limited its use. Blood Transfusion can be simple top up of additional units of blood without removal of sickle blood from patients, or exchange with the removal of sickle blood that is replaced by normal red cells. Material and Methods: This is a prospective study to be carried out at single centre Model Blood Bank, Department of Pathology, Dr. B.R.A.M. hospital Raipur (C.G.) India, over a period of 1 year from January 2013 to December 2013. All the subjects who has diagnosed Sickle Cell Anaemia and need for Blood Transfusion from them we collect all necessary clinico- haematological profile. Any Blood Transfusion Reaction (BTR) encountered would be recorded. All the clinical data related to blood transfusion was noted. Data was recorded in MS EXCEL and checked for its completeness and correctness then it was analysed. Results: There are 188 males (53.72%) and 162 females (46.28%). 188 male 523 (61.61%) blood unit transfused and in 162 female 326 (38.39%) blood unit transfused. Total 849 units of blood transfusion, 664 interval has occurred in one year. The hepatomegaly and splenomegaly were found in 208 (59.43%) and 129 (36.86%) patients, respectively. The indication for blood transfusion in sickle cell patients is 281 (80.29%) in acute condition and 69 (19.1%) is for planned condition. Indication for acute transfusion is 102 (36.30%) patients is for anemia, 97 (34.52%) patients is for acute painful episode, and 82 patients (29.18%) is for acute chest syndrome. Out of 350 Transfused Sickle Cell Anaemia Patients, 3 patients (0.85%) were found seropositive for viral markers. Out of 3 seropositive patients two (0.57%) was seropositive for HIV 1and2, one (0.28%) is seropositive for HBV and none of the patient was found seropositive for HCV. Conclusion: Blood transfusion is an effective therapy in the acute and chronic treatment of sickle cell disease.
