A STUDY ON THE USE OF FASTING C-PEPTIDE ASSAY IN DIABETES MELLITUS OCCURING IN THE YOUNG

Abstract
Aim : Classification of Diabetes Mellitus as Type1 or Type2 Diabetes at the time of diagnosis occurring in the younger individuals (Age < 25yrs) is difficult due to varied presentations. Insulin therapy depends on the type of Diabetes Mellitus. Hence, fasting c-peptide levels help in determining therapy with insulin. Materials and Methods : 50 patients who were diagnosed as Diabetes Mellitus of age at onset less than 25 years attending Diabetology OPD, at Government Stanley Hospital were studied. Detailed history and examination was done. Blood samples were taken after an overnight fasting for c-peptide levels. Results : Among the study population 38 patients had T1DM, 10 had T2DM and 2 had other specific types. 39 patients had c-peptide levels of <1.1nmol/L and 11 patients had c-peptide level of >1.1nmol/L. The mean c-peptide levels in T1DM is 0.61nmol/L and 1.73nmol/L in T2DM ( p=0.001). Conclusions : Estimation of fasting C-Peptide level showed a statistically significant role in identifying young diabetes subjects who require Insulin therapy.
