Application progress of middle cerebral artery, umbilical artery, uterine artery and cerebroplacental ratio in risk assessment of fetal distress in uterus
Journal Title: Journal of Air Force Medical University - Year 2023, Vol 44, Issue 3
Abstract
Fetal distress in uterus refers to a series of pathological changes such as abnormal fetal heart rate and acidosis caused by acute or chronic hypoxia of fetus in uterus due to various reasons. If not discovered in time, it will cause serious consequences such as neonatal asphyxia, abnormal neural tube development and even death. With the development of medical imaging technology and the safety and repeatability of Doppler ultrasound, the application of color Doppler ultrasound in obstetrics is becoming more and more meticulous and precise. When fetal distress in uterus occurs, all blood flow indexes of umbilical artery are higher than those of normal fetus, and UA-S / D > 3 is of great significance to parturients with gestational age greater than 32 weeks. The middle cerebral artery is more sensitive than the umbilical artery, and its indexes have higher Youden index for pregnant women of 28 ~ 36 + 6 weeks. For pre-eclampsia patients, there is a certain correlation between the change of uterine artery resistance and the severity of the disease. As a relatively new predictive index, the value of cerebroplacental ratio is still controversial. This review mainly discusses the application progress of color Doppler ultrasound in monitoring various indexes of fetal hemodynamics, and evaluates its significance for timely detection of fetal distress in uterus and improvement of pregnancy outcomes.
