Basic EMS teams facing patient with irreversible cardiac arrest

Abstract

Introduction. Emergency Medical System Enactment created Basic EMS teams, which do not have a physiscian as their member. Such teams are being dispatched also to patients with irreversilbe cardiac arrest, or those already dead.Aim. Aim of the thesis is to find out, how do basic EMS teams deal with such situations, and how does the use of specialized EMS teams as backup units influence local cirizens’ safety and availability of the EMS. Analysis of legal regulations considering abovementioned procedures has been done, and proposal of changes has been submitted.Material and methods. Retrospective research is based upon medical documentation created by 16 basic and 14 specialized EMS teams of Kraków Ambulance Service in 2012. 356 medical cards considering clinical or confirmed death of 173 individual patients have been researched. This led to selection of 94 cases in which basic EMS team has withdrawn from resuscitation. Procedures undertaken by these teams have been examined. Accumulative time of work done by specialized EMS teams in order to issue death certificate has been described, along with amount of distance travelled.Results. Result has shown, that basic teams assess their patients correctly, and undertake appropriate steps. It is however a problem to make use of specialized team only to issue death certificate, especially in areas where few such teams are based. Legal regualtions need to be updated. Allowing basic team to perform wider range of procedures, including issuing death certificate, is worth considering. Since there are few forensic medicine specialists in Poland, there is a need to establish qualifications necessary to become a coroner.Conclusions. There are plans to update Emergency Medical System Enactment soon. Findings of the thesis may contribute to creation of efficient and reliable Emergency Medical System in Poland.
