Birinci Basamak ve Yataklı Kamu Sağlık Kurumlarının Gelirlerinden Yapılan Ödemelerde Performans Yönetimi Kavramı

Abstract
Bu makalede, kamu tarafından sunulan sağlık hizmetlerinde döner sermaye gelirlerinden dağıtılan performansa dayalı ek ödeme sistemlerini tanımlamak amaçlanmıştır. Sağlık hizmetlerinde performans esaslı döner sermaye gelirlerinin dağıtımını düzenlemek amacıyla çıkan tüm mevzuat ayrıntılı olarak incelenmiştir. İncelenen sağlık mevzuatı örneklendirilerek hekimlerin ve sağlık personelinin konuyu örneklerle ayrıntılı olarak kavraması amacına yönelik bir yöntem izlenmiştir. Hastanelerde döner sermayeden dağıtılan ek ödemeler için kullanılan performans yönetimi, hekimin ve hemşirenin yapılan işlem karşılığı ödeme aldığı şeklinde yorumlanmaktadır. Aslında, ne kadar fazla işlem yapılırsa yapılsın hastanenin aylık gelirlerinin sadece %50’si bazı kurallara göre dağıtılır. Sağlık personeline dağıtılacak toplam %50’lik döner sermaye miktarı, hastanenin kalite katsayısı ile çarpılır ve bu çarpım sonucu azalan döner sermaye miktarı dağıtıma tabi olur. Döner sermayeden performans ek ödemesi yapılacağı zaman, o hastanede çalışan tüm hekimlerin ortalama puanı alınmaktadır. Döner sermayede toplanan paranın kaynağını Sağlık Uygulama Tebliği paketleri oluşturur. Bu paketler içinde sadece tedavi giderleri vardır; ancak amortisman, elektrik, su, ısıtma vb. bedelleri yoktur. Aile Hekimliği Merkezi’nde (ASM) ve Toplum Sağlığı Merkezi’nde (TSM) henüz performansa dayalı ek ödemeler yapılmamakla birlikte yakın gelecekte bazı uygulamalar için yapılması beklenmektedir.
