Comparative analysis of mouthrinses efficacy when used as plaque inhibitory agents

Abstract

Introduction. Chemical plaque control plays a supportive role in the prevention and treatment of gingivitis. Most antiseptics are introduced in the form of toothpastes and mouthwashes, and their role involves reducing the presence of supragingival plaque and independent anti-inflammatory effect. Aim of the study. Randomized and blinded clinical evaluation of the effectiveness of non-alcoholic mouthwashes containing essential oils (Dentofresh ®) and aminofluoride (Elmex Sensitive Plus ®) used as a supplement to brushing in adult patients with gingivitis caused by plaque only. Materials and methods. The study included 84 patients with gingivitis aged from 17 to 48 years (mean 29.5). Patients were randomly assigned to 3 groups: group A – 34 persons were given oral hygiene instructions and were recommended to brush twice a day with the triclosan toothpaste and 4 weeks of oral rinse twice a day for 1 min with Dentofresh®, study group B – 25 persons – hygiene instructions were given and they were instructed to brush twice a day with the triclosan toothpaste and 4 weeks of oral rinse twice a day for 1 min with Elmex Sensitive Plus, control group C – 25 people – hygiene instruction and recommendation were given to brush three times a day with triclosan toothpaste. The following clinical parameters were evaluated: indicators of oral hygiene (PCR according to O’Leary and API according to Lange et al.), indicators of gingival inflammation (BOP by Ainamo and Bay, and PBI by Saxera and Mühlemanna), the calculus index CI according to Ennevera and tooth discoloration index DI by Lobene. The survey was conducted four times: before randomization and after 2, 4 and 8 weeks after it. Results. In all groups significant reductions in the values of both indicators of plaque in subsequent controls in relation to first evaluation were reported. The greatest reduction in both hygiene indicators was found after application of rinse with essential oils. All three treatments led to a significant decrease in both indicators of inflammation in relation to the initial survey. The greatest reduction in the intensity of inflammation was observed after using mouthwash with essential oils. Substantial increase in the prevalence of calculus on the last visit, were recorded only in the control group. Only in group A, there was no significant increase in the size and intensity of discoloration of the teeth on subsequent visits. Conclusions. Inhibition of supragingival plates and anti-inflammatory activity were confirmed mainly for alcohol-free mouthwash with essential oils.
