Comparison Of Distal Tibia Fracture Fixation Techniques - Conventional Plating Vs Mipo

Abstract
Background: The management of unstable distal tibia fractures remains Challenging. The proximity to the ankle makes the surgical treatment more complicated. There are many fixation techniques to treat the fracture,Conventional open reduction and internal fixation (ORIF) techniques involve extensive dissection and periosteal stripping, which increase the risk of soft tissue complications.The minimally invasive plate osteosynthesis (MIPO) technique has gained prevalence in recent years. This technique uses indirect reduction methods and allows stabilization of distal tibia fractures while preserving the vascularity of the soft tissue envelope. Objective: The aim of this study was to compare the results between two surgical options for distal tibia fracture, i.e. minimally invasive plate osteosynthesis (MIPO) vs. open reduction and internal fixation (ORIF), and explore the advantages and pit falls of these two techniques. Material and Method: In our study, we retrospectively analysed Thirty cases of distal tibia fracture, 15 of ORIF and MIPO each were studied in between August 2012 to July 2015 and evaluated in terms of operative time, blood loss, healing time, time of recovery to work, implant irritation symptoms, and union status. AOFAS score was used for functional evaluation. Result: No malunion occurred and one case developed infection in the ORIF group.In the ORIF group, Ten cases were evaluated as excellent, three as good, two as fair. In the MIPO group, Nine cases were excellent and five good and one as poor. In conclusion, the MIPO technique is not better than ORIF in treatment of distal tibia fracture.
