Comprehensive Study Of Intertrochanteric Fracture Management Using Proximal Femoral Nailing

Abstract
Background: Intertrochanteric fractures are the most common fractures seen in the elderly age group. Considering the increase in life expectancy and relative increase in geriatric age group, volume of these fractures has also increased. Early restoration of ambulation is the key in the management of such patients. Unstable fractures pose significant challenge as most of these patients are osteoporotic. Aim of this study was to evaluate the efficiency of proximal femoral nail (PFN) in the management of intertrochanteric and peritrochanteric fractures by preventing varus collapse and early restoration of ambulation. Methodology: A prospective study of intertrochanteric and peritrochanteric fracture management using PFN was conducted. A total of 50 patients above the age of 60 were evaluated. Few improvisations were made to manage unstable intertrochanteric fractures in addition to conventional surgical technique. Functional outcome was assessed using Harris hip score. Results: All patients were successfully treated with PFN. These patients had minimal blood loss, minimum need of post operative analgesia and minimum hospital stay. 92 percent of our patients had excellent scoring by 6 months. Conclusion: Our study revealed that proximal femoral nail is far superior implant in the management of all types of peritrochanteric fractures. PFN being a weight sharing implant provides better stability of the fracture there by helps in early ambulation of elderly patients. Improvisation in surgical techniques makes PFN superior in the management of unstable intertrochanteric fractures.
