Endovascular mechanical thrombectomy of primary and recurrent changes in the arteries and bypasses – preliminary own experience

Abstract

Introduction. Atherosclerosis is the most common disease of lower extremity arteries and a main cause of chronic limb ischemia. Treatment options of advanced peripheral arterial disease include surgical methods (endarterectomy, bypass) and endovascular techniques which currently are increasingly used. One of endovascular treatment technique is a mechanical thrombectomy or atherectomy.Aim. To evaluate effectiveness of treatment of primary and secondary changes in arteries and bypasses with using endovascular mechanical systems.Material and methods. Between January 2011 and April 2014, a 26 mechanical recanalization of arteries/bypass graft were performed. In 19 patients (16 men, 3 women) the procedures were performed with SilverHawk/TurboHawk systems, while in 7 patients (6 men, 1 woman) Rotarex system was used. In the follow-up period (3-24 months) the Doppler examinations were performed 1, 6, 12 and 24 months after the procedure. During the Doppler examination the blood flow in treated vessel/bypass was assessed.Results. Technical success of all procedures was 96.2%, 100% for the systems Silver/TurboHawk (Covidien/Medtronic) and 85.6% for the system Rotarex (Straub Medical). There were two serious complications in the study group (2/26 treatments – 7.6%) – 1 perforation of the artery and 1 peripheral embolism, minor complications were observed in 3 patients (11.5%). In the 12-month follow-up period there were 1 in stent restenosis and 1 bypass graft thrombosis.Conclusions. The preliminary results of endovascular treatment with using mechanical thrombectomy systems show that this method is an interesting alternative in treatment primary changes in the arteries, the recurrent lesions, and occluded bypass, however this outcome should be confirmed in a larger group of patients.
