EVALUATION OF THE OPTIMAL PARAMETERS FOR EXCITATION OF MONOSYNAPTIC REFLEX ELEMENTS ARC IN STUDIES OF H-REFLEX FOR ELECTROTHERAPY PURPOSES

Abstract
Introduction Reports of world literature on new techniques and effectiveness of electrotherapy applied to nerves and muscles estimate it at about 60% (Dimitrijevic 2008). Functional diagnosis (studies of electroneurography and electromyography) and the clinical picture of the patient may influence for the type of electrotherapy procedures, the selection of safe and effective nerve stimulation parameters adapted to the functional capabilities of the damaged structures in the nervous and muscle systems. Aim The aim of the study was to determine the optimal intensity of electrical pulses and their frequency during the selective electrical stimulation of sensory Ia fibers in the median nerve based on the examination of individual parameters of H-reflex in healthy volunteers. Material and methods Twenty one healthy volunteers underwent the sEMG study (surface electromyography) from the abductor pollicis brevis muscle to determine the average value of the amplitude of the electromyogram before and after selective electrical stimulation applied to group Ia afferent fibers of the median nerve. Selection of electrical stimulation parameters was based on analysis of induced H-reflex from abductor pollicis brevis muscles after electrical stimulation of the median nerve. The duration of single electrical stimulation session was set at 10 minutes, algorithm of stimulation series consisted of a train of three individual electrical monopolar pulses with a duration of 0.5 ms at interval of 7 ms and frequency of about 20 Hz. Results The change of sEMG amplitude parameter recorded from the abductor pollicis brevis muscle, comparing its average value from three tests before and after electrical stimulation. The average value of the amplitude significantly increased at about 405μ v (p = 0.05). Conclusions The study verified the hypothesis that an electrical pulse of precisely established selective parameters applied to the group Ia afferent fibers in median nerve causes a change in the activity of motor neurons, resulting in the muscle contraction parameter changes recorded with sEMG method.
