Helicopter emergency missions for patients with severe body injuries in 2011-2012 in the operational district of Helicopter Emergency Medicine Service in Białystok

Abstract

Introduction. Treatment of multiple body injuries frequently require cooperation between specialists in different surgical areas which may be guaranteed mainly in trauma centers. Helicopter Emergency Medical Service is able to reach in short time every region of the country and transport patients with severe body injuries do proper unit, i.e. trauma center.Aim. The aim of the study was to describe the cross-section of patients admitted to Trauma Center transported by Helicopter Emergency Medical Service, specificity of this type of transport and to estimate costs and efficiency of helicopter disposition.Material and methods. In present study, medical documentation of Helicopter Emergency Medical Service branch in Bialystok and Emergency Department has been analysed. Research has been conducted between September 2011 and December 2012. Results underwent statistical analysis using descriptive methods.Results. In analyzed material, 64 from 103 Helicopter Emergency Medical Service (HEMS) interventions regarded patients with multiple body injuries. 10 patients suffered from burns, 7 were diagnosed with isolated severe craniocerebral injury, 22 patients have been transported due to isolated injury and remained in good health state. 39 patients have been transported to Trauma Center of University Hospital in Bialystok. Among them, 2 met the criteria for admission to Trauma Center, recognized at the place of accident. Among patients admitted to Trauma Center, 14 required immediate surgical treatment and/or treatment of post injury collapse. The median of the time of reaction “on the ground” in cooperation with Emergency Medical Service (EMS) was 14 minutes while the median of the time of reaction of HEMS was 19 minutes. Median of the time of air transport was 17 minutes while the median of expected time of road transport from the place of accident was 57 minutes. The median of patient transport to proper center from the moment of receiving the report by the team of EMS was 63 minutes.Conclusions. Air transport significantly shortens the time of patients transfer to the hospital. Verification of criteria established for patients transported to trauma centers seems to be appropriate as well as monitoring the system in order to eliminate cases when patients are qualified improperly. It is suggested that teams of EMS should be trained in fast recognition of patients which require air transport and in efficient cooperation which lead to shorter time of patients transfer to Helicopter Emergency Medical Service teams. Gradual extension of HEMS duties in selected centers in the country is worth considering along with analysis of the proportion of incomes and costs of air transport of patients with severe injuries.
