Immune and anti-inflammatory factors in the mechanism of therapeutic effect of metformin in type 2 diabetes mellitus (analytical review including the results of own researches)

Abstract
The review deals with the analysis of modern literature, including the results of own researches on the mechanism of the therapeutic effect of metformin in patients with type 2 diabetes mellitus (DM2) and its complications. Despite the fact that a vast number of patients are treated with metformin, there are a large number of publications around the world in which its clinical effect is discussed, the mechanism of the curative action of metformin remains unclear. The paper presents new data indicating that not only the hypoglycemic effect of metformin plays an important role in the mechanism of the curative effect in DM2 and its complications, but also its favorable effect on inflammation and immunoregulatory processes disturbed in DM2, as evidenced by a decrease in the inflammatory index (N/L index), normalization of the quantity and function of lymphocytes of various immunophenotypes and reduced levels of proinflammatory cytokines and chemokines in the blood.
