Intramedullary fixation of humeral fractures and non-unions of humerus

Abstract
Humeral fractures are considered as a difficult clinical problem. Such fractures had been treated non-operatively for many years and early trials with plate fixation were often followed by non-union. The creation of intramedullary nailing, has successfully contributed to the treatment of humeral fractures. It was also quickly and successfully employed in treatment of humeral non-unions. This technique allows for fixation of almost all types of humeral fractures. The intramedullary nailing is more patient friendly”, than classical, conservative methods. Intramedullary nailing allows a full range of motion, starting the next day post surgery. It gives the opportunity for faster employment of physical therapy and quicker restoration of previous physical activity. Additionally, it gives the opportunity for treatment of multifragment or multilevel fractures with lower frequency of non-unions or malunions as compared to the plate fixation. Introduction of novel materials and methods allowed for application of intramedullary nailing not only for humeral shaft fractures but also fractures of proximal humerus. In our article we review indications for intramedullary nail fixation. We will weigh methods of treatment for humeral fractures and present clinical vignettes. We also discuss complications after intramedullary nailing of humerus.
