Obturacyjny bezdech senny jako intedyscyplinarny problem medyczny

Abstract

Obstructive sleep apnea syndrome (OSAS) is a major medical problem in several – dozen or so percent of the population of industrialized countries. In most, but not always, occurs with excess body weight and metabolic syndrome. The survey conducted among physicians that OSAS is underdiagnosed and neglected, identified primarily with simple snoring. This paper examines the co-occurrence of OSAS with other diseases or disorders of homeostasis. Among the 162 patients were men and 41 women. Average age of men 50.69 (SD 12.2) and women 58.28 (SD 11.4) differed significantly (p < 0.001). In 49 (24.14%) patients (39 men and 10 women) the AHI score: 5.1–14.9 was obtained, which allowed them to identify with mild obstructive sleep apnea. In 51 (25.12%) patients (40 men and 12 women) were diagnosed with the moderate (AHI 15.1–29.3) and in 103 (50.74%) patients (83 men and 19 women), with severe (AHI 30.1–115.7) OSAS. Overweight was diagnosed in 69 patients (33.99%) and obesity in 65 (32.02%). Hypertension was present in 54 patients (26.6%). An interview revealed that some patients had recognized and treated arrhythmias stated in the 7 patients (3.02% of all patients). Among arrhythmias in patients with OSA were: paroxysmal bradycardia (1), paroxysmal tachycardia (1), paroxysmal atrial fibrillation (4), panic disorder ventricular arrhythmias (1). In 2 patients (0.99% of all patients), only with severe OSAS diagnosed with dilated cardiomyopathy, and one (0.49% of all patients) mitral regurgitation. Ischemic stroke have gone 4 people (1.97% of all patients), including one twice. Heart attacks have gone 3 people (1.48%) and coronary artery disease suffered 6 people (2.96% of all patients). Episodes of venous thrombosis/ pulmonary embolism occurred in 4 patients (1.97% of all patients). Statistically significant differences as far as fasting hyperglycemia, atherogenic index >5 and the prevalence of obesity are concerned was proved between patients with mild to medium severity of OSAS, comparing with a group of patients with severe OSAS, with a level of probability = 0.001. In the summary it was found that cardiovascular disease, excessive weight, impaired glucose and lipid metabolism are common problems in patients with OSAS; severity and clinical stages of obstructive sleep apnea corresponded to the studied morphofunctional parameters.
