Platelet concentrates, as new and promising agent in the orthopedic surgery – an introduction

Abstract

Tissue healing is a complex process, involving numerous interactions between cells, that are mediated by signal molecules, including growth factors. The commonly used source of growth factors in orthopedics and trauma surgery are platelet concentrates, also calle platlet rich plasma (PRP). It is obtained by centrifugation of patient's whole blood, and contains increased levels of growth factors such as PDGF, TGFβ and other. Commercially available sets of separators allow for easy and fast application of platelet concentrates, even in outpatients departments. It is important know what criteria characterize obtained platelet concentrates and influence its efficacy in clinical use. The indications of use of growth factors in orthopedics started with treatment of impaired bone healing. Nowadays clinical data are available documenting successful use of PRP in pseudarthrosis, bony reconstructions, dysplasia fibrosa, joint replacement surgery, ostitis, arthrodesis. Recent year brought up data about use of growth factors from platelet concentrates in sport injuries and soft tissues surgery as anterior cruciate ligament reconstruction, meniscal surgery, cartilage repair, treatment of muscle and tendon injuries. The goal of this article is to give a review of contemporary indications and possible ways of administration of PRP, as well as to show criteria that allow to compare commercially available systems of platelet concentrates.
