Precancer vulvar dysplasia – a literature review and self-experience

Abstract

Introduction. Precancer vulvar dysplasia similarly to precancer cervical dysplasia was classified as VIN 1, VIN 2 and VIN 3. Nowadays, according to pathological changes it has been divided into uVIN and dVIN. For many years the Chair and Department of Gynecology, Obstetrics and Oncological Gynecology had been treating precancer vulvar dysplasia and vulvar cancer. Less invasive treatment is sought due to a decrease in patient’s age e.g. CO2 laser treatment.Aim of the study. Assesment of the accuracy of the primary diagnosis of precancer vulvar lesions and correctness of qualification to operation performed at the Chair and Department of Gynecology, Obstetrics and Oncological Gynecology MUS in Bytom on the basis of current literature.Material and methods. We present 12 randomly selected patients with diagnosed vulvar pathology. Our group consisted of 3 patients with VIN 2, 3 with VIN 3, 3 with Paget’s disease, 2 women with vulvar cancer and 1 with chronic vulvitis.Results. The performed operations included: 3 simple vulvectomies, 1 posterior vulvectomy, 1 deep vulvectomy, 1 unilateral labial resection, 4 tumor excisions and 2 vulvar biopsies. Women were offered as conserving therapy as possible because of difficult and long-lasting healing process in perineal area.Conclusions. Our clinical practice demands early introduction of vulvar cancer prophylaxis including smoking counseling and regular check-up in HPV positive patients.
