Precancerous conditions in gynecology

Abstract

Precancerous conditions in gynecologic oncology are the changes where malignant neoplasms occur more frequently than in the healthy tissue of female genital tract.Vulvar and vaginal precancerous leasions occure in any age of women. Signifficant increase of their occurence is observed in relatively young patients (i.e. below 40 yrs.). Ethiopatogenic factors are: HPV infections, chronic inflammation, altered tissue trophy. Diagnostisc of vulvar and vaginal leasions is always based on specimen pathological examination. Treatment depends on the pathologic result as well as clinical status of the patient.Cervical cancer has been a serious epidemiological problem for many years in Poland. The number of cases gradually decreases but it still remains at too high rate.Cervical Intraepithelial Neoplasia (CIN) generally appears in the transformation zone (transient zone), nearby the external orifice. Leasions may progress into invasive cancer or can stay fixed for many years. Among numerous well known cervical cancer development risk factors, the most significant is the infection of human papilloma virus, especially with typical, high potential oncogenic virus. Treatment depends on severity of the changes. The majority of uterine carcinoma is preceeded by abnormal proliferation.The vast maiority of endometrial cancers is preceeded by pathologic hyperplasia with atypia of the endometrium. However it is noteworthy, that approx. 1.6% of endometrial carcinoma is not proceeded by atypia of the endometrium. In case of endometrial hyperplasia with atypia the progression to endometrial carcinoma is observed in 23-56% of all cases. The mode of endometrial hyperplasia treatment depends on precise type of the leasion.
