PROGNOSTIC FACTORS OF ACUTE ISCHEMIC STROKE IN POSTERIOR CIRCULATION FOR ASSESSMENT OF ITS CLINICAL COURSE AND OUTCOMES

Abstract

Knowledge of clinical peculiarities of the course of ischemic strokes in posterior circulation (PC), early identification of unfavorable outcome factors will improve the results of treatment of ischemic strokes and to predict clinical consequences. 145 patients were involved in the study with acute PC cerebral infarction. All patients were hospitalized during the first 6-24 hours after the appearance of the primary symptoms of a stroke. According to the recovery degree of neurological functions, using the modified Rankin scale at 21 days of treatment, all patients were divided into two groups: 1-st group (53 patients) had unfavorable functional outcome (3-6 points) and the 2-nd group (92 patients) – favorable (0-2 points). Demographic baselines, risk factors for ischemic stroke, basic neurological deficit and dynamics of its recovery, the results of laboratory tests were analyzed in both groups in details. Significant improvement of neurological deficits in patients of the 1-st group was recorded on the 14-th day of observation with the help of NIHSS and B. Hoffenberth et al. scales, while among patients in the 2-nd group – already on the 7-th day. The factors, affecting the unfavourable functional outcome of patients with ischemic stroke in PC on the 21-st day of observation, were determined. A statistically significant difference of indicators in both groups were found for such factors as: the severity of the neurological deficits by NIHSS and B. Hoffenberth et al. scores (p<0.001), the degree of consciousness by Glasgow coma scale (p <0.001), presence of atrial fibrillation (p = 0.03), cardioembolic stroke subtype (p = 0.004), the presence of transient ischemic attacks in anamnesis (p=0, 02), the combined lesion of different PC structures (p<0.001).
