Rapid palatal expansion in late growth phase and after the growth completion- part I (a review)

Abstract
The aim of our study was to rationalise data related to the effectiveness of methods for palatal suture opening in patients after 15 years of age. Reports concerning clinical trials of the usage of the rapid maxillary expansion after the growth spurt and also histological and radiological maturity of palatal suture depending on age were presented. The review was carried out based on an indexed bibliography between 1953 – 2010, from medical data bases and the hand searching at medical libraries. Twenty four papers were selected from the literature and they were used as source in our study. According to the majority of authors, obliteration of the palatal suture prevents dehiscence of palatal processes of the maxilla and so it requires surgical intervention. With the use of rapid maxillary expansion the inclination of alveolar processes can be achieved and it might be accompanied by some complications like loss of bone, root resorption and gum recession. The other group of authors consider the possibility of orthopaedic maxilla expansion alone. The researchers compare histological examinations and the radiological maturity of palatal suture. They also checked the effect of rapid maxillary expansion on the periodontium depending on activation protocol and the type of appliance. The main conclusion is that, until now there is no alternative pattern for surgical procedure for adult patients with maxillary constriction. (Orthod. Forum 2011; 7: 78 - 84).
