ROLE OF ULTRASOUND IN MANAGEMENT OF BLUNT ABDOMINAL TRAUMA

Abstract

BACKGROUND Blunt abdominal trauma is one of the most common causes of death in young population. It is a diagnostic challenge, even to the experienced surgeons. Timely management is very important to prevent death due to blood loss. With the development of Ultrasonography (USG) it is possible to evaluate and manage the patients with BAT promptly. METHODS Present study conducted on patients with blunt abdominal trauma admitted to surgery department, Hi-Tech Medical College, Bhubaneswar during the period 2016-2018. Patients were referred to the Department of Radiology and Radiodiagnosis for transabdominal ultrasonography using 3.5-12 MHz transducer. All cases were critically evaluated and correlated with clinical background, laboratory investigations, conventional and radiographic procedure, CT scan and operative findings wherever possible. RESULTS Road traffic accident was the major cause. US was able to identify free fluid, solid organ injuries (lacerations, contusions, hematomas and rupture) and perivisceral fluid collection. Incidence of hollow viscous perforation is more than visceral injuries. Amongst the visceral injuries, spleen, liver and kidney were the most common abdominal organs injured with incidence of 17.5%, 10% and 2.5% respectively. CONCLUSIONS Ultrasonography is not only cheap and non-invasive but is also rapid, portable for bedside evaluation in the emergency room, without any radiation to patient, requires no administration of contrast medium and can be repeated at any time without contraindication.
