Severe resorption of maxillary incisors as a result of orthodontic treatment – case report

Abstract
Orthodontic therapy carries a risk of complications. One of them is the problem of root resorption. This is a common phenomenon. However, extreme cases like the one presented in this study are very rare. Resorption occurs as a result of many factors, but the process itself has not been fully comprehended so far as it is demonstrated by differing resorption rates in patients treated in health care establishment using the same techniques. [b]The aim of the stud[/b]y is to present the problem of severe root resorption as a complication after orthodontic therapy discussing the case of a female patient treated for a period of four years and eight months with fixed appliances. [b]Material and methods[/b]. Extraoral and intraoral examinations and plaster model analysis were performed, panoramic and cephalometric radiographs were evaluated prior to and after the treatment, as well as the CBCT outcome. The length, vitality, and mobility of the roots as well as pain symptoms in the maxillary incisors were evaluated. [b]Results[/b]. When analyzing available medical documentation, the following findings were made: a significant shortening of the maxillary incisor roots, an increased inclination of the maxillary and mandibular incisors and the formation of infraocclusion caused by orthodontic treatment. The roots of the upper incisors were shorter by more than 1/3 of their lengths as compared to the state presented in a panoramic radiograph performed prior to the therapy. The root resorption rate can only be assessed with the use of a 3D CT examination. [b]Summary.[/b] In the presented case the following factors were found to be responsible for resorption to occur: time and method of orthodontic treatment, trauma of the frontal part of upper jaw incurred before orthodontic treatment, and taking antiepileptic medication.
