STUDY OF MATERNAL AND PERINATAL OUTCOME IN BOOKED VERSUS UNBOOKED PREGNANT WOMEN IN A RURAL BASED MEDICAL COLLEGE

Abstract

BACKGROUND Antenatal care is the care of woman during pregnancy. Also known as prenatal care, it is preventive healthcare, with the goal of providing regular check-ups that allow doctors or midwives to treat and prevent potential health problems throughout the course of the pregnancy while promoting healthy lifestyles that benefit both mother and child. The purpose of this study was to analyse the factors influencing the utilization of antenatal care services. We wanted to compare the maternal and perinatal outcome in booked versus unbooked cases in a rural based hospital. MATERIALS AND METHODS This is a prospective observational study conducted at Santhiram General Hospital, Nandyal on 300 antenatal patients. The instrument for data collection was self-developed questionnaire based partly on the antenatal registration book provided by Government of Andhra Pradesh. Structured questionnaire was administered to cases and it was divided into sections relevant to the objective of the study. A detailed history was obtained which included sociodemographic status, previous and present obstetric history, chief complaints for which she was admitted and pregnancy related complications, and history of any medical and surgical illness was obtained. The data obtained was analysed using statistical package for social sciences (SPSS) version 22. Level of significance was set at 5% (0.05) such that significant associations were established when p <0.05. RESULTS The main objective of the study was to assess the factors influencing the utilization of antenatal care services by the patients and the maternal and perinatal outcome in booked versus unbooked cases. The incidence of anaemia is increased in unbooked cases (25%). Booked patients were also found to be having less chances of HDP (6.66%) when compared to unbooked patients (25.9%). The incidence of GDM and foetal growth restriction is almost same in both the groups. The preterm labour incidence was more in unbooked cases (11.8%) compared to booked cases (4.2%). The incidence of perinatal complications was higher in unbooked cases. The incidence of IUFD (2.2%), foetal weight less than 2.5 Kgs (61.7%), prematurity (74%), early neonatal deaths (71.4%) was higher in unbooked cases as compared to incidence of IUFD (0%), foetal weight less than 2.5 Kgs (38.3%), prematurity (26%), early neonatal deaths (28.6%) in the booked cases. The rate of normal delivery was almost equal in either groups whereas unbooked cases show increased rate of caesarean section (58%) compared to booked cases (42%). 0.3% of maternal mortality was observed in unbooked cases whereas it was nil in booked cases. CONCLUSION This study concludes that various factors influence the utilization of antenatal care services. The most prominent factors that influenced the compliance of the patients were parity, area of residence, education status of women, socioeconomic status of the patients. The present study clearly reflected that booked pregnant women had early diagnosis and appropriate management with fewer complications during the pregnancy and better maternal and perinatal outcome compared to unbooked women
