The Evaluation of The Impact of Pulse Corticosteroid Therapy on Bone Mineral Density in Multiple Sclerosis Patients

Abstract

Objectives: Osteoporosis is one of the most common complication of long term corticosteroid therapy. Pulsed methylprednisolone therapy is proceeded several times to multiple sclerosis(MS) patients as attack treatment throughout their lives. Little is known about the effect of this treatment on bone mineral density (BMD). In order to asses this aspect of treatment, we evaluated the BMD of patients who have taken pulse steroid therapy versus MS patients without any steroid therapy. Material and methods: Fiftythree patients were included in the study. The patients who received methylprednisolone at least 10g during the last two years, or at least 15g during the course of the whole disease were assigned as treatment group (n=32). Twenty one additional MS patients who did not receive pulse steroid served as controls.All of the patients were ambulatory (EDSS<4). We evaluated Blood calcium, inorganic phosporus, alkaline phosphatase, parathormon levels and urine Ca/creatinin ratios, lumbar BMD, femur neck BMD, lumbar total T-score, femur neck T-score, lumbar Z-score, femur neck Z-scores of the treatment and the control groups. Results: We found no meaningful relationship between the groups for lumbar BMD, femur neck BMD, lumbar total T-score, femur neck T-score, lumbar Z-score, femur neck Z-score (p=0,317; p=0,78; p=0.54; p=0.77; p=0,59; p=0,94 respectively). Blood calcium, inorganic phosporus, alkaline phosphatase, parathormon levels and urine Ca/creatinin ratios were similar between the two groups (p=0.07; p=0.87; p=0.77; p=0.32; p=0.57 respectively). Conclusions: Our results showed that pulsed steroid therapy had no effect on BMD of MS patients.
