Transmission of drug-resistant TB among family members

Abstract
Introduction. Tuberculosis is an infectious disease transmitted by droplet. In controlling the spread of the disease the most important is early detection of smear-positive patients, implementation of appropriate treatment and tracing the chain of transmission of infection. Many years of research on tuberculosis transmission in the environment of patients indicate, that the risk of infection among close contacts, especially family members, is very high. Therefore, epidemiological investigation should be extended to all persons who have contact with the TB patient and under special review should be the people living in the environment with patients with drug-resistant tuberculosis.Material and methods. The subject of the study were drug-resistant M. tuberculosis complex strains isolated from 10 patients who were members of 4 families. In one of the families father and three grown children were identified, and in the remaining three families pairs of TB patients were identified – mother and 12-year-old son, mother and adult daughter and a marriage coming from Chechnya. In the Department of Microbiology IGiChP drug resistance patterns of strains were verified and investigated their genetic relatedness. The three-stage typing analysis, molecular methods have been used: spoligotyping, IS6110-Mtb1 Mtb2 PCR and MIRU-VNTR.Results. Among the 10 strains tested, identified 4 strains resistant to SM, 2 INH-resistant, 2 resistant to INH + SM-resistant and 2 resistant to RMP + INH + SM + EMB + OFL (pre-XDR resistance) were identified. Analysis of DNA patterns indicated the genetic relatedness of strains isolated from patients within each family.Conclusions. The observation of identical patterns of drug resistance and the same DNA profiles among strains isolated from family members confirmed the phenomenon of transmission of drug resistant tuberculosis among patients.
