Type and efficacy of materials used in alveolar osteoplasty in patients with primary palate cleft

Abstract
Introduction. Alveolar bone grafting has become a standard in the treatment of cleft lip and palate. The main objectives of this procedure are: to allow the eruption of teeth into the cleft and the orthodontic movement of teeth adjacent to the cleft side, give support to the lips and the alar base, eliminate the oro-nasal fistula, enhance maxillary segments stability, provide bone for subsequent placement of implants.Objectives. The aim of this study was to evaluate the efficacy of different graft materials in patients with cleft lip and palate based on the review of the literature.Material and Methods. We used the key words: cleft lip and palate and alveoloplasty, guided bone regeneration, autografts, allografts, xenografts, alloplastic materials, growth factors, barrier membrane in different combinations for searching through the literature in the “PubMed” database. We identified a total of 40 publications describing the efficacy of different grafts in patients with cleft lip and palate.Results. Autografts were the most widely examined group. The success rate of this graft material was high and depended on many factors: status of eruption of the canine, the type of cleft, patient’s age. The failure was reported more frequently in the case of bilateral cleft lip and palate, when the grafting was done after the eruption of the canine, in older patients and in patients with poor oral hygiene.Conclusions. The highest success rates for alveolar bone graft were achieved in patients with cleft lip and alveolar process with or without cleft palate when bone grafting was carried out before permanent canine eruption by using autograft from the iliac crest.
