Comparative evaluation study between cytological and histopathological grading of Breast carcinoma

Abstract
Background and Objectives: Assigning cytological grade to breast carcinoma guide neo adjuvant chemotherapy and prognosis. To grade the breast carcinoma on Fine needle aspiration cytology using Robinson grading system and to assess the concordance of cytological grading (CG) with histological grading (HG) using Elston‑Ellis modification of Scarff‑Bloom‑Richardson grading system. Material and Methods: A retrospective study was done on 54 cases of breast cancer reported and graded on cytology for 1 year. After mastectomy all cases were graded histopathologically. Correlation between cytological and histopathological grading system was determined. Sensitivity and specificity of Robinson’s cytological grading system was calculated in each grade. All cases evaluated for presence of metastasis to axillary lymph nodes. Result: Out of 54 cases graded by Robinson cytology grading system, 16 cases were grade I, while 30 and 8 cases were graded as II and III respectively. The concordance rate between cytological grade I and histology grade I was 87.5%.while for grade II and III was 73.33% and 75% respectively. All cases of grade III were positive for metastasis in axillary lymph node. While grade II and I was 80% and 18.75% respectively. Conclusion: Cytological grade is helpful in selecting neoadjuvant chemotherapy and predictor of prognosis. Robinson’s grading system of breast carcinoma is simple, quick and correlates well with histological grade with higher grade associated with regional lymph node metastasis.
