Endovascular treatment of diabetic foot syndrome

Abstract

<b>Introduction.</b> Diabetic foot syndrome is an infection and/or ulceration and/or destruction of deep tissue foot (skin, muscle, bone) caused by damage of peripheral nerves or limb arteries. It affects 5-20% of patients with diabetes and is the leading cause of amputation of the lower limbs in developed countries. In connection with increasing incidence of diabetes, also diabetic complications become the serious problem of contemporary medicine.<br><b>Material and methods. </b>In the period from January 2009 to September 2010 in the Department of Radiology, Medical University of Lublin, 100 procedures of lower limb arteries angioplasty were performed in 92 patients (aged 55-91) diagnosed with diabetic foot syndrome (bilateral in 8 cases). Due to the presence of ulcers, tissue loss or necrosis, all patients were classified on a scale of Rutherford as the fifth and the sixth category. Technical success of procedure was determined in four point scale: very good and satisfactory when procedure improved blood flow to the foot, lack of improvement after an unsuccessful attempt or deterioration.<br><b>Results. </b>Very good or satisfactory technical success rate was achieved in 90% of cases, no improvement was noted after 8 procedures, in 2 cases deterioration occurred. Control examinations was performed at 1 and 6 months after procedure, during follow up examinations healing of feet changes were assessed. Complete healing of ulceration after 6 months was achieved in 38 cases, partial in 26, small amputation was performed in 28 patients, eight times high amputation was necessary.<br><b>Conclusions.</b> The results of this study show high efficiency of endovascular treatment in patients with diabetic macroangiopathy and good healing of the trophic changes after the successful restoration of blood flow to the feet. Endovascular treatment is the last chance to save the limb before amputation.
