Variations of the thoracic duct on its termination

Abstract

[b]Introduction: [/b]The thoracic duct is the collector trunk of all the lymphatic vessels, excepts for the right half of the head, neck and thorax. It begins in the superior part of the abdominal cavity, crossing the diaphragm through the aortic orifice, reaches the posterior mediastinum, goes along the thorax and when it reaches the base of the neck on the left side, it ends in the jugular subclavian confluence. In this study, we describe the anatomic variations of the thoracic duct outlet in human cadavers. [b]Materials and Methods:[/b] Fifteen human corpses were reviewed, and we selected those who had the thoracic duct intact and located near the left jugular subclavian confluence, along with lymphatic ganglia that drained in the thoracic duct or other lymphatic vessels that allowed its identification. We proceed to dissect and locate the thoracic duct, and to outline the site of its opening.[b]Results: [/b]Of fifteen cases, 11 had the outlet site in the jugular subclavian confluence, two ended in the subclavian vein, one in the internal jugular vein, and one in the external jugular vein.[b]Conclusions: [/b]Variations of the thoracic duct in human corpses exist, of which the most frequent is the opening in the subclavian vein. Also the existence of a non described variant in the literature was observed, corresponding to the opening in the external jugular vein. These variations must be considered at the time of approaching this area in medical procedures in order to not injure the thoracic duct.
