Utility of lipid measurements in early detection of oral malignancy

Abstract
Background: Oral cancer is the fifth most common cancer in the world with more than 3,00,000 new cases diagnosed every year. In India it ranks first among all types of cancers in male patients. India has recorded the highest incidence of oral cancer where it accounts for 40-50 % of all cancers. Aims and Objective: To study the utility of lipid measurements in early detection of oral malignancy. Materials and Method: In the present study three groups were formed containing 50 individuals each. Group I (oral precancerous lesions): in this group of fifty patients diagnosed with oral precancerous lesions were selected. Group II (control group): Fifty age and sex matched healthy controls were also included. Serum levels of lipid profile comprising of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), very low density lipoprotein cholesterol (VLDL-C) were measured in all the individuals of the three groups. Result: The mean age of subjects in control group was 45.64±9.06 years while the mean age of patients of oral precancerous lesions. The number of male in oral precancerous lesions were 29(58%) while in controls it was 30(60%). The mean value of total cholesterol (mg/dl) in controls, cases of oral precancerous lesions 177.14 ±13.93mg/dl and 145.36 ± 10.06mg/dl. The mean value of Triglyceride (mg/dl) in control group was 113.54±11.58mg/dl while that in oral precancer was 112.64±14.26mg/dl. The mean value of LDL cholesterol (mg/dl) in controls, cases of oral precancerous lesions was found to be 112.66 ± 14.93 mg/dl and 105.38 ± 11.01 mg/dl respectively. The mean value of VLDL (mg/dl) in control group was 23.62±2.40 mg/dl while that in oral pre-cancer was 22.54±2.96mg/dl. The mean value of HDL Cholesterol (mg/dl) in controls was found to be 41.72±6.18mg/dl and in oral precancer it was 34.72±6.66mg/dl. Conclusion: The findings of present study points towards hypolipidemia in patients of precancerous lesions. The change in lipid levels may have a diagnostic or prognostic role in the early diagnosis and treatment of oral premalignant and malignant lesions. Further studies along these lines may be useful for the prediction of prognosis and response to cancer treatment.
